Finding a Provider Within Florida


Can I go to an eye specialist? I have HMO coverage and I live in South Florida?

If you live in certain counties and are enrolled in an HMO plan, you or your routine eye doctor must contact Eye Management, Inc. at 1-800-329-1152, Option1, before you receive ophthalmology services. This is the company that authorizes ophthalmology services and can refer you to a participating doctor for coverage under your HMO plan. Please review the counties below to determine if this applies to you.

 


How do I find all participating radiology / imaging centers for a specific service such as an MRI, CT or PET scan?

How do I find all participating Urgent Care Centers/Walk-in Clinics?

How do I find lactation counseling services in Florida?

Lactation (breastfeeding) counseling services are usually provided by midwives, obstetricians and facilities that offer delivery services, such as hospitals or birthing centers. Talk to your obstetrician, midwife or your baby's pediatrician to see if they provide lactation counseling. Or, use the Find a Doctor feature above to search for providers in your network.


Do I have to select a Primary Care Physician (PCP)?

BlueCare - Yes. Select a participating Primary Care Physician from our BlueCare HMO network. To change your Primary Care Physician online, go to My Account, Benefit & Claims Tab and select Change Primary Care Physician
SimplyBlue - Yes. Select a participating Primary Care Physician from our SimplyBlue HMO network. To change your Primary Care Physician online, go to My Account, Benefit & Claims Tab and select Change Primary Care Physician
BlueChoice - No. With a BlueChoice health benefit plan, you are free to see any physician, without a referral. However, you can minimize your costs by selecting physicians who are General Practitioners, Internists or Family Physicians and who participate in our Preferred Patient Care (PPO) provider network. Visit Find a Doctor and More to see if your physician matches these criteria.
BlueOptions/BlueSelect - No. With the BlueOptions and BlueSelect health benefit plans, you are free to see any physician, without a referral. However you can minimize your costs by selecting physicians who are General Practitioners, Internists or Family Physicians and who participate in NetworkBlue or the BlueSelect Network. Can I change my Primary Care Physician any time I want?

Yes. You may designate a new PCP at any time. To change your PCP online, go to My Account, Benefit & Claims Tab and select Change Primary Care Physician


Finding a Provider Outside of Florida


How do I find a participating pharmacy outside of Florida?

On our pharmacy website, select your pharmacy network from the drop down menu and enter a 5-digit zip code to find participating pharmacies outside of Florida. Note: Log on to our member site, Floridablue.com, for personalized pharmacy benefits and cost information.


How do I find a doctor or hospital outside the United States, Worldwide?

Please use our Worldwide Doctor and Hospital Finder directory to locate participating providers outside of the United States, Worldwide.


How do I find a doctor or hospital outside of Florida within the United States?

Please use our National Doctor and Hospital Finder directory to locate participating providers outside of Florida within the United States.


Questions About Your Dental Plan


I am a BlueDental Care member. Can I visit a dentist who is not a member of the BlueDental Care prepaid network? No. The BlueDental Care plan is an in-network plan only. There are no out-of-network benefits.
Where can I find a list of participating dentists for my plan?

Visit www.floridabluedental.com and look under the Find a Dentist heading at the top of the page. You can search by name or location and even print a customized directory of general dentists by county. You can also call Customer Service at (888) 223-4892 from 8 a.m. to 8 p.m., Monday through Friday for BlueDental Choice members or (877) 325-3979 from 8 a.m. to 6 p.m. Monday through Friday for BlueDental Care members to speak with a representative who will help you find a dentist.


I am a BlueDental Care member. I’d like to switch to another dentist. How do I do that? You can change your dentist by calling Customer Service at (877) 325-3979 from 8 a.m. to 6 p.m, Monday through Friday.
How can I add or remove a dependent from my Dental plan?

BlueDental Choice

BlueDental Care members should call Customer Service at (877) 325-3979 from 8 a.m. to 6 p.m., Monday through Friday to add or remove dependents from your plan.

 


I need out-of-area emergency dental care. Does my Dental plan cover it?

For BlueDental Choice, yes.  Coverage may be considered out of network.

For BlueDental Care, yes. If you’re more than 100 miles from the nearest available participating general dentist, you can obtain reimbursement for expenses for emergency care rendered by any licensed dentist, less applicable company copayments, up to $100 per member, per year, upon presentation of an itemized statement of emergency services from the dental office that provided the care. Florida Combined Life must be notified of any emergency treatment within 90 days of receipt of care.


Is temporomandibular joint disorder (TMJ) covered by my BlueDental Choice plan? No. Treatment for TMJ is not covered under the BlueDental Choice or BlueDental Care plans.
Are there any limitations and exclusions under my Dental plan?

Please refer to your policy or certificate of coverage for plan exclusions and limitations.


I am a BlueDental Choice member. Is there an online tool that will help me manage my dental benefits and view claims?

Yes. Go to floridabluedental.com  and access MyBlueDental.  You can view and print Explanation of Benefits statements, review covered services and claims, check your annual maximum and benefit amounts used to-date and more.


I am a BlueDental Choice member. Can I switch from an individual PPO Plus plan to an individual PPO Copay plan? Yes. You can switch from one plan to the other if you do it within 30 days. After 30 days, you will need to wait until your plan anniversary date. 
I’m a new Dental member. Can I keep my same dentist?

If you have BlueDental Care and your dentist participates in our BlueDental Care prepaid network, you may continue to see him or her. If your dentist is not a member of our BlueDental Care prepaid network, you can nominate him or her by calling (877) 325-3979 from 8 a.m. to 6 p.m., Monday through Friday.

 

If you have BlueDental Choice you will receive maximum benefits and be protected from balance billing when you use a participating provider in the BlueDental Choice and Choice Plus (PPO) or BlueDental Choice Copayment (PPO) networks.  If your dentist is not a member of our BlueDental Choice and Choice Plus (PPO) or BlueDental Choice Copayment (PPO) networks, you can nominate by calling 888-223-4892 from 8 a.m. to 8 p.m., Monday through Friday or by emailing us at fclprovidernomination@fclife.com.


If I have questions about my enrollment status, benefits or billing, who do I call?

BlueDental Choice:
